Dysmenorrhoea means painful cramping pain with mustruation.
There are Two Types:
Primary dysmenorrhoea is menstrual pain associated with ovular cycles in the absence of pathologic findings. There would be no any identifiable pelvic pathology. The pain is attribute to biochemical derangement. The pain usually begins within 1-2- years after the menarche and may become more severe with time. It affects more than 50% of post-pubescent women in the age groups of 18 to 25 years.
Secondary dysmenorrhoea is menstrual pain for which an organic cause exists. It usually begins well after menarche, sometimes even as late as the third or fourth decade of life. The organic causes are like – fibroids, adenomyosis, PID ( pelvic inflammatory disease), endometriosis etc.
There are three clinical varieties:
Spasmodic dysmenorrhoea: is the most prevalent and manifests as cramping pains, generally most pronounced on the first and second day of menstruation.
Congestive dysmenorrhoea: manifests as increasing pelvic discomfort and pelvic pain a few days before menses begin. Thereafter the patient rapidly experiences relief in her symptoms. This variety is commonly seen in pelvic inflammatory disease or pelvic endometriosis.
Membraneous dysmenorrhoea is a special group in which the endometrium is shed as a cast at the time of menstruation. The passage of the cast is accompanied by painful uterine cramps.
What are the clinical symptoms of dysmenorrhoea ?
Pain in lower abdomen.
Uterine cramping.
Dragging pains in lower limbs.
Nausea and vomiting.
Headache and feverishness.
Fainting and seizures.
Clotted bleeding.
Extreme weakness.
Backache.
Polyurination.
Diarrhea.
Gastric disorders etc.
What are the Investigations?
Pelvic sonography followed by CT scan or MRI scan if indicated.
Physician physical examination.
